TACKLE REGISTRATION INCLUDES:

Millard United Issued Equipment: Helmet, Shoulder Pads,  Practice Jersey, Practice & Game Pants.  Game Jersey is issued and kept by the player. 

A $300 Equipment Deposit & $45 Equipment Cleaning Deposit  is required.  You are completing this registration to secure your equipment & cleaning fee deposits with a credit card.  As long as you return your equipment & it is clean prior to the date listed below, nothing will be deducted from your credit card account.  However if you do not return your equipment or it is not clean, the deposit amounts will be deducted from your credit card account.  This registration must be completed before equipment is released.

Equipment must be returned & clean prior to December 16th.
