Please use this registration to complete the Request for Scholarship Application.   

Please attach verification of your approval for your school district's free or reduced lunch program to have your request for scholarship of your player's registration fee reviewed. 

If you cannot provide MUS with your free or reduced school lunch eligibility letter, you may submit your Educational Benefits Eligibility Letter or documentation showing your Supplemental Nutrition Assistance Program (SNAP), Temporary Assistance for Needy Families (TANF) or the Food Distribution Program on Indian Reservations (FDPIR) benefits for review. 

**Requests must be received one week before Late Registration begins to be considered**

**Requests may take up to two weeks to be processed**
